To trigger Johanka's DLC you need to complete 'In God's Hands' and talk to her, then complete the 'Pestilence' quest. that will trigger the DLC. I did, I even asked her about it, but when it came to the "Start this DLC now?" question I said no and did Theresa's DLC first. Then when I came back from Theresa's DLC Johanka was gone.
